Ongoing Case Management & Rehabilitation for Children and Young People
We are specialists in supporting children, young people, and their families following life-changing injuries and trauma. With years of experience in complex case management, we understand how to provide responsive, personalised rehabilitation that adapts as children grow — emotionally, physically, and socially.
Our approach to ongoing case management centres around one simple truth: children and young people aren’t just participants in their care — they are partners in shaping it in whatever way that works for them.
The Voice of the Child: Not just at assessment stage
Our commitment to making the voice of the child as loud as it can be travels from assessment through to ongoing case management. We revisit the Immediate Needs Assessment and plan goals rehabilitation around the child and families changing needs.
We continuously work to understand what matters most, recognise fears, hopes and preferences, include them in decisions and respect their identity, interests and boundaries. We bring this to life through age-appropriate tools and meaningful engagement, ensuring that children and young people feel seen, heard, and empowered.
Tools That Empower: My Rehabilitation, My Voice
To help children and young people express themselves and be heard, we use ‘My Rehabilitation, My Voice’ — available in two versions:
- For ages 0–10: A creative, playful guide that helps younger children share their feelings, identify their support networks, and talk about their goals in ways that are meaningful to them.
- For ages 11–18: A more mature and reflective version that supports teenagers in exploring their story, identifying challenges, setting goals, and asserting their preferences and rights. It encourages self-advocacy and builds confidence in making choices about their own rehabilitation.
We also use:
- Child- and young people friendly check-ins to monitor wellbeing and progress
- Simple but powerful feedback tools, including surveys and emotion charts
- Developmentally appropriate support planning that evolves as needs change
Specialist Case Management That Grows With Them
Our team brings extensive experience in paediatric case management, neurorehabilitation, and trauma-informed care. We work holistically with families, clinicians, schools, and community providers to co-create tailored support plans.
We advocate. We adapt. We build trust. And we never forget that healing happens best when children feel safe, understood, and involved. Because for us, the measure of success is not just recovery — it’s empowerment. Every voice matters. Every future deserves to be shaped by the child at its centre.
