Holistic Rehabilitation Pathways: How Caroline helped a young man move forward after a work injury

To be truly client centred and holistic, at Bush & Co, we explore what is important to the client and what they want to achieve at the end of their rehabilitation. We advocate for each client to ensure their views and priorities are heard whilst offering a clinical view point and suggestions to ensure rehabilitation goals are attainable.
As a case manager is it important to consider the client's clinical needs but to also apply a wider perspective and consider all areas that that are important to the client's rehabilitation. This includes looking at all potential barriers that could affect a client and how these may interact with each other.
By being holistic, we can not only ensure that a client’s rehabilitation goals can be met but it also helps us to anticipate potential barriers for the client and mitigate these where possible. We are able to facilitate a coordinated, smooth and consistent rehabilitation programme to support the client to meet their goal and optimise the experience and outcome.
Caroline Scarrow, Physiotherapist and Bush & Co Associate Case Manager, demonstrates what a holistic approach to case management looks like in practice. At just 21 years old, a life-altering work accident changed everything for Caroline's client. A severe brachial plexus injury left him without use of his upper limb, making daily tasks, work and recreational activities far more challenging. Yet, with an incredible support system and an unshakable drive, he was determined to regain his independence and redefine his future.
A Future Marked by Uncertainty: The Challenges He Faced
Following his injury, the client had several immediate concerns:
- Adjusting to life with a non-functioning upper limb
- Returning to work in an alternative role, as his previous job was physically demanding
- Re-engaging in pre-incident home and leisure activities
- Managing psychological impacts, including anxiety and stress
- Exploring potential medical interventions, including plastic and cosmetic surgery
Building a Strong Foundation: Caroline’s Approach
As his case manager, Caroline took a proactive approach, ensuring that all aspects of his rehabilitation were carefully managed and aligned with his personal goals.
Psychological Support
Understanding the mental impact of his injury, Caroline arranged private psychological support. Although he initially struggled with severe stress, anxiety, and depression, his proactive mindset and engagement in therapy led to remarkable improvements. At the end of this intervention, all of his psychological scores had returned to normal levels.
Occupational Therapy & Adaptations
An occupational therapist conducted a functional assessment to explore compensatory techniques and adaptive equipment to maximise independence. The client quickly engaged with the therapy and was eager to implement strategies that allowed him to manage personal care tasks independently. With all necessary advice and equipment in place, OT input was successfully completed.
Physiotherapy & Rehabilitation
Physiotherapy was a critical component of his recovery, involving both gym-based training and hydrotherapy. These sessions focused on maintaining range of motion, preventing complications, and exploring any potential for upper limb function recovery. Caroline ensured seamless communication between his therapy team and medical specialists to keep treatment plans aligned.
Vocational Rehabilitation & Career Redirection
With his previous physically demanding role no longer an option, Caroline coordinated a vocational assessment and ongoing career support. The client remained motivated and engaged, exploring new career possibilities suited to his abilities. Caroline maintained regular communication with his vocational case manager, ensuring he received the best guidance to transition back into the workforce.
Disability Support & Benefits
Recognising the financial impact of his injury, Caroline facilitated a benefits review to secure appropriate disability support. This provided a crucial safety net while he worked towards returning to employment.
Medical Coordination & Future Surgical Options
Given the severity of his injury, there was potential for further medical intervention. Caroline liaised closely with his NHS surgeon, keeping the team informed of his progress and ensuring he was considered for plastic and cosmetic surgery options in the future.
Outcomes: A Story of Progress and Determination
The impact of this structured and well-coordinated approach was evident in the client’s progress:
- Successfully returned to driving independently
- Psychological wellbeing and fatigue levels restored (all screening tool scores within normal limits within four months and maintained thereafter including fatigue and emotional stability)
- Secured full-time employment in a new, suitable role
- Returned to all pre-incident leisure and recreational activities
- Actively participating in physiotherapy to maximise recovery potential
Looking Ahead
While there are still steps to take on this journey, this young man’s determination, combined with the right support from Caroline and his rehabilitation team, has laid a solid foundation for his future. With a return to work on the horizon and continued progress in rehabilitation, his story is one of resilience, adaptation, and a refusal to let his injury define his life.
