For many, the start of a new year is a time of reflection and resolutions. Looking back on the past year at challenges that they may have faced, setting goals for the year ahead and establishing the foundations of how they may achieve these.
Goal setting can be a powerful motivator and its significance is also recognised in clinical settings. We’ve all heard the phrase, a dog is for life and not just for Christmas, well in case management , goal setting is not just for the new year and is instead a key part of their rehabilitation journey, happening at any time in the day, month or year.
Goal setting is well established in case management practices and in many ways needs no explanation. Case managers focus on goals from the outset, working with clients to establish what is important to them and helping them to break these goals down into achievable steps.
So what does this look like in practice? For one of Georgie Payne’s clients, an 87 year old male who lived an extremely active lifestyle, the goal was independence. Let’s take a look at how that one important goal was broken down into achievable steps…
Rebuilding Independence One Goal at a Time
A skilled carpenter by trade and a passionate cyclist, Georgie’s client would regularly complete 60-mile rides three times a week—and even continued racing. That all changed he sustained a C5 ASIA D spinal cord injury. Suddenly, the man who had spent his life building and creating found himself in a hospital bed, dependent on others for the simplest tasks.
After nearly a year in hospital and rehabilitation, he was discharged to a nursing home as his home was unsuitable for a wheelchair. Though initially reliant on a hoist transfer, his relentless motivation saw him progress to using a return transfer aid with assistance. But for a man used to a life of independence, this was only the beginning of his journey.
Setting Goals for Independence
With Georgie’s support, his case management plan focused on clear and achievable goals:
- Progressing from a return transfer aid to a step transfer, increasing his independence.
- Enhancing efficiency with daily functional tasks like shaving and brushing his teeth, reducing reliance on care staff.
- Improving hand control and coordination to enjoy his lifelong hobby of painting with greater precision.
- His long-term goal was to move out of the nursing home and return to independent living with an appropriate care package.
A Tailored Rehab Plan
To make these goals a reality, Georgie implemented a structured rehabilitation plan, ensuring the right support and resources were in place.
- Physical Rehabilitation Physiotherapy: A tailored stretching and strengthening program was introduced to reduce contractures, improve sit-to-stand transitions, and support his transfer goals.
- Rehab Equipment: The client independently arranged a trial of a MotoMed exercise bike, working on both upper and lower limb strength. Georgie pursued funding through the rehab code to secure permanent access to this valuable piece of equipment.
- Occupational Therapy and Daily Function Hand Function Therapy: Daily OT sessions targeted improved dexterity and strength to enhance independence in grooming and hobbies like painting.
- Pressure Management: Given his high risk of pressure sores—having already developed some in the hospital—Georgie ensured continuous assessment of his seating and pressure relief equipment.
- Enhancing Mobility and Access Taxi Account: Set up to enable regular outings to visit family and maintain social connections, which was vital for his mental well-being.
- Manual Wheelchair Assessment: His powered wheelchair presented accessibility challenges in certain community settings. A manual wheelchair assessment was arranged to improve his ability to navigate his daughter's home and other locations.
- Spasms Management: Increased spasms in his lower limbs were addressed by liaising with the NHS spinal injury service. His medication was adjusted, resulting in noticeable improvement.
- Technology and Communication Assistive Technology: Due to limited hand function, he struggled to use a phone, leaving him unable to call or text relatives. The Aspire charity’s assistive technology team advised on suitable devices and a new phone was set up, allowing him to stay connected with loved ones.
- Emotional and Psychological Well-Being Support: Initially struggling to adjust to his trauma and new limitations, he underwent therapy. His progress was positive, leading to discharge from psychological services after demonstrating no clinical signs of anxiety or depression.
Preparing for the Future
With independence on the horizon, Georgie worked closely with him to lay the groundwork for his next steps:
- Care Needs: Conversations with the NHS CHC case manager confirmed that his care needs could be met at home. When the time comes, a package of care will be implemented with additional private top-ups if necessary.
- Housing: He is in the process of selling his current home. Once completed, Georgie will support him in finding and adapting a suitable new residence.
- A Special Family Event: One of his greatest aspirations is to attend a special family event taking place in Ireland. Plans are underway to coordinate travel, accommodation, and care needs to ensure he can be there for this important milestone.
A Journey of Strength and Determination
Thanks to Georgie’s comprehensive case management, this resilient gentleman has gone from being completely dependent on care staff to regaining a significant degree of independence. With continued support, he is on track to achieving his ultimate goal—leaving the nursing home and reclaiming his life on his own terms. A goal not just for the new year but for a lifetime.
